starting April 4, 2014 at 6:00pm

 

Log in at http://smsportal.dlsl.edu.ph/StudentPortal/  to view your grades; use Internet Explorer in accessing the portal.

IMPORTANT REMINDERS

 

  1. Make sure that you have a copy of your grades Issued by the OCR. This copy of grades will be presented to the ocr during THE RELEASE OF YOUR OFFICIAL ENROLLMENT & ASSESSMENT fORMS (oEAF). yOU WILL ALSO BE REQUIRED TO PRESENT YOUR SIGNED/APPROVED PRE-ENROLLMENT FORMS
  2. ENROLLMENT OF DISMISSED STUDENTS WILL BE CANCELLED AUTOMATICALLY EVEN IF PAYMENT HAS BEEN MADE ALREADY

NOTE: Students Accounts in eWiz will be activated at 5:00pm, a day before the scheduled adjustment for the processing of changing of enrolled subjects in the student portal.

 

For ALL LAMP SCHOLARS except ACADEMIC SCHOLARS (you need to secure Scholarship Status Clearance Form first from the LAMP Office before claiming your OEAF)

 

 

PROCESS:

 

1.    Present your grades for the second semester of SY 2013-2014 and the approved Pre-Enrollment Form (PEF) to the Registrars staff. And for scholars except Academic Scholars, you need to present your SCHOLARSHIP STATUS Clearance Form. The OCR will not ISSUE OEAF OF STUDENT WHOSE PEF is ALTERED OR WITH CORRECTIONS

1.1           Registrars staff will verify if the enlisted subjects in the OEAF are the same subjects enlisted in the approved PEF

1.1.1      If the list of subjects in the PEF did not tally with the list of subjects in the OEAF, you need to reprocess PEF. It should be signed and approved by your Academic Adviser

 

1.2. The Registrars staff will issue the OEAF if the subjects in the OEAF tally with the subjects in the PEF. Secure 2 copies of your Official Enrollment and Assessment Form

 

2.    Proceed to the Accounting Office or to any of the following authorized banks: SM Bills Payment, China Bank SM Lipa, BPI at any branch nationwide, Robinsons Bank Lipa, RCBC Savings Lipa & Planters Bank Lipa for the payment of fees on scheduled dates below:

3. Proceed to the Registrars Office; present Official Receipt for ID validation.

 

NOTE:
