CLEARANCE & GRADES ISSUANCE for 1ST SEM of AY 2014-2015

 

ONLINE VIEWING of GRADES

1.    Log in at http://smsportal.dlsl.edu.ph/StudentPortal/  to view your grades starting 6:00pm of October 16 to October 17, 2014; use Internet Explorer in accessing the portal.

 

IF YOU ARE NOT YET CLEARED FROM THE FOLLOWING OFFICES,  ONLINE VIEWING WILL BE BLOCKED & COPY OF YOUR GRADES WILL NOT BE RELEASED:

b.    REGISTRARS OFFICE

c.     ACCOUNTING OFFICE

d.    COLLEGE and other ADMIN OFFICES

 

2.    Secure printed copy of your grades at your assigned COMPUTER LABORATORY on October 17, 2014 from 8:00am to 5:00pm only; refer to your Computer Laboratory assignment below:

Issuance of grades after October 17, 2014 will be at the Office of the Registrar, Windows 8 or 10 with payment.

 

IMPORTANT REMINDERS

 

1.      you are not authorized by the office of the registrar to share your password to anyone except to your parents or guardian.

 

2.      Make sure that you have a COMPLETE copy of your grades (NO BLANK OR INC GRADES) Issued by the OFFICE OF the REGISTRAR. This copy of grades will be presented during THE SCHEDULED RELEASE OF YOUR OFFICIAL ENROLLMENT & ASSESSMENT fORMS (oEAF), YOU ARE ALSO REQUIRED TO SUBMIT SIGNED/APPROVED PRE-ENROLLMENT FORMS.

2.    Present your grades for the 1st semester of SY 2014-2015 and the approved Pre-Enrollment Form (PEF) to the Registrars staff.

 

 

All scholars should present SCHOLARSHIP STATUS Clearance Form issued by the LAMP Office.

 

 

 Registrars staff will verify if the enlisted subjects in the OEAF are the same subjects enlisted in the approved PEF

a.       If the list of subjects in the PEF did not tally with the list of subjects in the OEAF, you need to reprocess PEF. It should be signed and approved by your Academic Adviser.

 

 

 

 

The Registrars staff will issue the OEAF if the subjects in the OEAF tally with the subjects in the PEF.

 

3.    Secure 2 copies of your Official Enrollment and Assessment Form.

 

 

 

 

PAYMENT OF TUITION FEES

       Proceed to the Accounting Office or to any of the following authorized banks for the payment of fees on scheduled dates below for the payment of fees:

      Note: For payment through accredited banks (inside and outside

    Final Process

                      

    For students who paid in DLSL cashiers directly:

 

After payment, proceed to the Registrars Office to present Official Receipt for ID validation and OEAF stamping.

 

    For students who paid in the accredited banks (inside & outside DLSL):

 

After 3 working days, proceed to the Registrars Office to present Deposit Slip for ID validation and OEAF stamping.

 

Note:

 

If you cannot personally claim your grades, process enrollment, get copies of OEAFs and request for documents, your representative should bring an Authorization Letter, photocopy of your school ID and valid ID of your representative to be presented to the OR staff.

 

 

 

ENROLLMENT OF DISMISSED STUDENTS WILL BE AUTOMATICALLY CANCELLED EVEN IF PAYMENT HAS BEEN MADE ALREADY
