ENROLLMENT PROCESS AND REMINDERS FOR 1ST SEM 2016-2017

 

 

Step 1

Proceed to your designated Academic Adviser. Present your latest semestral grades and PEF (Pre-Enrollment Form) with your proposed subject loads for his/her approval.

 For LAMP Scholars:  please make sure that you renew your scholarship with the LAMP Office on your scheduled date.

 

Step 2

 

Present the approved PEF and latest grades to the assigned staff and secure Enrollment and Assessment Form (EAF)

 

Assigned staff will verify number of failures as reflected in the latest grades presented and validates the enrolled subjects of students in eWIz  vis- -vis the Academic adviser's approved PEF, recomputation of tuition fees will also be done in the Ewiz Sytem.

 

Step 3

 

Proceed to the Cashier's Office at the College Lobby for the payment of your tuition and fees.

 

Before leaving the Cashier's Office, please make sure that your EAF is stamped enrolled by the Cashier's staff.

 

Please take note of this policy on payment of tuition and fees:

 

    a.  Payments made on or before June 10, 2016 will not incur penalty.

    b.  Payments made from June 13 to 17, 2016 will incur a penalty of P150.  
    c.  Payments made from
June 20 to 24, 2016 will incur a penalty of P 200.

    c. Payments made after June 24, 2016 will not be accepted.

    d. Bank payments will be accepted up to June 23, 2016 only

 

Step 4

 

Proceed to the Office of the Registrar for the validation of your ID Cards and validation of your enrollment. 

Students whose ID Cards are not validated after June 13, 2016 will not be allowed to attend classes.

 

REMINDERS

1.  STUDENTS ARE NOT ALLOWED TO ATTEND CLASSES IF THEY ARE NOT YET PAID AND OFFICIALLY LISTED IN THE CLASS LIST.

 

2.  LAST DAY OF PAYMENT AT THE DLSL CASHIER FOR THE FIRST SEMESTER IS UNTIL JUNE 24, 2016 ONLY.  Students are not allowed to pay outside DLSL after June 23, 2016.  Payments will not be recorded in DLSL and students will not be officially enlisted.

 

3. NO STUDENTS WILL BE ALLOWED TO PROCESS THEIR ENROLLMENT AFTER JUNE 24, 2016.

 

     3.1 Pre-enrolled subjects will be deleted on June 27, 2016 if not yet paid on or before June 24, 2016.

 

4.  FACULTY MEMBERS ARE REQUIRED TO SECURE THEIR OFFICIAL CLASS LIST FROM THEIR RESPECTIVE SECRETARIES ON JUNE 13, 2016.

5.  FACULTY MEMBERS ARE NOT ALLOWED TO ACCEPT STUDENTS WHOSE NAMES ARE NOT IN THE OFFICIAL CLASS LIST.

 

6. TUTORIAL CLASSES - Payment for Tutorial Classes will be up to June 24, 2016 ONLY.   Please see your respective Registrar's Coordinator (temporarily assigned at MB 509, 409 and 510) for the processing. 

7.  SPECIAL CLASSES.  Computation for Special Classes will be accommodated up to June 17, 2016 ONLY.  Application for Special Classes will be accommodated starting May 2, 2016.  Please make sure that you have the Schedule of Classes in coordination with your Department / Area Chair before you enlist other students to avoid conflict in their subject loads for the first semester.
