1. Submitted grading
sheets with Incomplete grades can only be
completed through the Grade Completion
Form.
2. The faculty member must complete the Grade
Completion Form after the student has submitted
the requirement for
completion or has taken the special final
examination.
3. The Grade Completion Form must be signed
by the Department Head and the College Dean
for submission to
the OCR.
4. In case the student did not submit the
requirement or did not take the special final
exam on the scheduled
date, the subject teacher must compute the
student's grade giving the missed requirement
or examination a grade of 50% or 0%.
5. The deadline for submission of Grade Completion
Form to the OCR is two weeks after the issuance
of grades to students.
6. Practicum/Internship course teacher, who
shall require an extension for submission
of grade, should inform
the OCR before the deadline of the submission
of grades.
7. Student's grade shall automatically become
5.00 or failure if the course teacher fails
to submit the Grade
Completion Form on time.
