For SENIOR students: 4th year under the Five Year Degree Programs; 3rd year under the Four year Degree Programs, and 2nd Year under the TwoYear Certificate Programs

1. Visit the OCR on the exact date and time that you chose for the scheduled evaluation of your     academic records. The evaluation will be done to determine the remaining courses that you     need to enroll in order to graduate in March.

2. Proceed to your program coordinator for the evaluation proper. Refer to the assigned program     coordinator per program below:

3. Secure a copy of the summary of the courses that you have taken and determine the remaining     courses that you need to enroll in order to qualify for the next year's graduation.

4. After the evaluation, you will be informed as to your expected date of graduation provided that     all the remaining courses were enrolled and passed.

5. Secure a copy of your Evaluation Form duly signed by the OCR staff and the Registrar.

6. Give a copy of the Evaluation Form to your academic adviser. Keep the Evaluation Form and     present it during your enrollment.
