1. Schedule Of Activities
a. 1st Week of
July - Distribution of Application Forms
for Graduation to the graduating students
through the class leaders. Take note of the
following in completing the forms:
(1)
Attach a copy of 2x2 recent ID picture (in
formal attire with white background)
(2)
Indicate course loads for the 2nd semester
of by referring to appropriate
curriculum
(copies are available at the OCR)
(3)
Accomplish the Form completely
Graduation
is not automatic. Students who have completed
their programs need to apply to
graduate, which includes an evaluation of
records and payment of applicable fees.
b. 2nd Week
of July - Submission of fully accomplished
Application Form for Graduation at the
OCR.

Proceed to the assigned window and submit
the forms personally
(2)
Upon submission, Candidate for Graduation
Evaluation Slip will be given
(3)
Indicate preferred schedule for evaluation,
which will run for 10 minutes only
(4)
Sign at the Evaluation Schedule Sheet
(5)
Make sure to come on time for the scheduled
evaluation
c. July
to Aug (whole month) - Evaluation period
for all graduating students. Venue: Office of
the College Registrar; Time - 8:30 to 11:10
(ten students per day per program coordinator)

Academic records will be evaluated to ascertain
whether the student has fulfilled all the
other
requirements as discussed with them during
their undergraduate evaluation in the previous
years to qualify them to be candidates for
graduation.
(2)
Graduation checklist will be given on the
same day to inform students of their status/deficiencies.
(3)
Schedule of picture taking will be given
d. September
(whole month) - Picture taking of all
graduating students. Venue of picture taking will
be posted at the Registrar's Bulletin Board
two weeks before the scheduled date.
e. 1st Week
of December - Re-evaluation of the status
of the candidates for graduation based
on the previous evaluation of the subjects
taken and passed

Duly qualified candidates for graduation will
be determined by each program coordinator
f. 1st Week
of January - Posting of the Initial List
of Candidates for Graduation for checking
by the
Graduating Students (Accuracy of the entry
of names shall be checked)
g. 2nd Week
of January - Forwarding of all deliberation
forms to respective colleges
h. 3rd Week
of January - Initial Departmental Deliberation
of Graduating Students during the
Activity Period to be conducted by the respective
colleges
i. 4th Week
of January - Consolidation of deliberation
reports to be presented to the Council of
Deans
j. 4th Week
of January - Release of graduation pictures
to the graduating students
k. 1st Week
of February - Interview of the Candidates
for Graduation with failures (as per result
of Initial Departmental deliberation) to be
conducted by the Registrar. Announcements
relative to this activity will be posted at
the Daily Bulletin
l. 2nd Week
of March - Submission of Final Course
Grades of Graduating Students to Area Chairs/Department
Heads/Deans for CBEAM/ CNURSING/ CEAS/ CITE
Final Deliberation of Graduating
Students will be facilitated by OCR in coordination
with the Area Chairs/Department
Heads/Deans for CBEAM/ CNURSING/ CEAS/ CITE.
m. 2nd Week
of March - Submission of Final Course
Grades of Graduating Students to the
OCR. Determination and confirmation of student's
eligibility for graduation.
n. 3rd
Week of March - Posting of Final List
of Candidates for Graduation at the College
Registrar's
Bulletin Board
o. 3rd Week
of March - Graduation Practices (SENTRUM)
p. 4th Week
of March - Distribution of Programs and
personalized invitations (OCR)
q. 4th Week
of March - Baccalaureate Mass & Recognition
Day (SENTRUM) Distribution of
TOGA (NOLI Building)
r. March
(a day after the Baccalaureate Mass) -
Commencement Exercises at the SENTRUM;
Collection of TOGA at the CENTEN Sports Plaza
2. Candidates for graduation are given up
to the last week of August to inform
the College Registrar
of any change or correction in their names
appearing in the list of Candidates for Graduation.
For special cases wherein corrections have
not been made prior to graduation, graduates
are provided a grace period of two (2) months
from receipt of Diploma and a copy of Transcript
of Records to have errors corrected. However,
this request will be submitted to CHED
for information and correction of record.
