° Sealed recommendation form properly filled out and duly signed by any of the following: Principal, Class Adviser or Guidance Counselor

Transferees: Levels 2-6 (Grades 2-6)
° Photocopy of Birth Certificate
° Two (2) copies of the applicant’s 1" x 1" picture
° Properly and completely filled out application form
° Certified true or correct: photocopy of Form 138 (Report Card)
° Sealed recommendation form properly filled up and duly signed by any of the following: Principal, Class Adviser or Guidance Counselor

Transferees (Levels 7-9 First to Third Year High School)
° Photocopy of birth certificate
° Two (2) copies of the applicant’s 1" x 1" picture
° Properly and completely filled out application form
° Certified true or correct: photocopy of previous schoolyear’s report card with a final average grade not lower than 80% and a final deportment rating of at least B or its equivalent
° Sealed recommendation form properly filled up and duly signed by any of the following: Principal, Class Adviser or Guidance Counselor
° Personal interview
